eCHRIS LEAVE REQUEST INTRODUCTION VIDEO
In April 2014, all City departments (except Police and Fire) will be switching to paperless leave request processing using eCHRIS. NEW FMLA FORMS ARE AVAILABLE
The Certification for Health Care Provider form has been replaced with two new forms. Which form to use is determined by whether your FMLA request will be for you or for a family member. Go to Empl / Supv Services to find these forms.
